no grave spawning when killed by unstable ingot

There seems to be a problem when you get killed by the unstable ingot (when the time goes out)
How to reproduce it:
Crafting 4 unstable ingots and placing them in the vanilla crafting table until the timer runs out then taking one ingot out kills you without spawning a gravestone and does not drop items.
this is happening in the latest Direwolf20 pack release
(extrautils-1.0.3c,OpenBlocks-1.2.7 and OpenModsLib-0.4b)
