I think this is the root cause of #17 and #18, Lithium is constantly generating new chunks for any OptiFine version newer than G3_pre4. For a test of the theory, try with mixin.world.chunk_access=false in the Lithium config.

I've spun up a custom Lithium build to see if this resolves all the problems if you'd like to give it a try here.
