These issues are caused by bugs exclusive to Origins (Forge), and have already been reported to the developers

Known Issues:

• Sometimes, origin descriptions cannot be seen
• Riding or picking up other entities can cause issues
• Deathsworn cannot obtain petrified hearts

(These are issues that affect the latest version of Origins and Origins++. If you are having issues, try updating to the latest of each first)
