For the log you can use https://gist.github.com. Just create a new gist and copy the file contents (preferably from the logs/latest.log next to your mods folder) into that. Then just provide the link to the gist in the issue.
Besides that, just fill in everything that is asked for. Currently, I don't even know which Minecraft version or modloader you are using.
