You seem to have ripped your license directly from MinecraftForge, despite the fact that the license literally cites the original within the first sentence... I'd recommend copying the text here into your LICENSE.txt if you wanted to license your mod under LGPL 2.1, or deleting LICENSE.txt if you didn't. Additionally, I'd recommend updating your README.txt to be more relevant.
