Hm, that's certainly a big issue. It might have to do with how the syncing is done, it's possible I could solve it temporarily by special-casing the Pattern Grid. I'll have to look into the code for how the Pattern Grid specifically operates. I'll try to have a workable resolution for you by the end of tomorrow.

I think temporarily it would also be nice to have a config to blacklist specific screens from the mod.
But I am not sure what could be used as identifier for each screen. You probably need a reference to their class.

I've hotfixed it by temporarily disabling the Polymorph integration for the Pattern Grid specifically. Ideally I'd like to make it work together, but that'll require some more time on my part and I'd rather address the exploit quickly rather than letting it sit for too long.

I consider this the right solution for right now as it's a pretty game-breaking behaviour. Thanks a lot!
