[1.16.5-3.67.7] Smelting Not Giveing Bonus Items

Describe the bug
With a high smithing skill, there should be chances at extra ingots being smelted from ore. This doesn't happen.
Expected behavior
With a skill of ~200 smithing, I should get bonus ingots.
To Reproduce
Steps to reproduce the behavior:
- Make new world
- Give 200 levels in
smithing - Place furnace, put stack of charcoal and 16 iron ore in
- Only get 16 ingots out
Versions:
- Minecraft: (eg 1.16.5)
- Loader: (eg. Forge-36.2.39)
- PMMO: 1.16.5-3.67.7
