This is a different dupe bug than #1521

Looking at certain angles you will get an extra item back. It is not mining a second block. Looking down at about 45 degrees seems to make the dupe pretty consistent.

This does not require any enchantments to reproduce.

It's consistent for me at about 51.5 degrees. Press F3 and it's on the same line as "Facing (north, south, east, west)" at the very end.

I'm getting a duplication issue pretty reliably with the pickarang. However, I only get it when I have a coin of fortune from Reliquary activated and the block I mine is close by.

I'm guessing this issue got closed because it is for 1.12.2?

I don't have that mod. You can replicate it with just quark installed but the angle seems to matter.
He probably closed it so that people would be less likely to know about it and exploit it. I just tested in 1.14 and had the same effect.
