Combining an enchanted book with an ancient tome does not raise the level of the enchanted book as intended. It instead just consumes 35 levels and returns the same enchanted book.

Are you combining the tome with an enchanted book of the same enchantment (e.g. Sharpness V tome + Sharpness V book)?

Hmmm... idk sorry. If there's a more recent version of the mod for 1.12, try it with that version

Are you using JustEnoughIDs by chance?
I had the same issue and it was caused by JustEnoughIDs on my end.
