Describing this specifically with Malum's brooches that add rune slots. When a totem of holding is retrieved, if the player has a brooch, their curios are going to be offset and misplaced in the wrong slots (rings into belt slot, belt into necklace and so on). Often the equipped runes are going simply disappear on retrieval.
