All players failing to connect after server restart

Issue description:
Players are disconnected with the message "A fatal error has occured, this connect is terminated" immediately after connection to server.

Steps to reproduce:
The issue started after a clean server restart. I do not know how it was produced.

Versions:

  • Minecraft: 1.12.2
  • Forge: 14.23.5.2775
  • McJtyLib: 1.12-3.0.7
  • RFTools: 1.12-7.58

Possibly incompatible mods and versions: FoamFix, BetterFPS

Removing RFTools and readding it fixed the issue temporarily, if I get the error again I'll try to find which mod is causing it.

Things to try:

  • See if you can reproduce this with no other mods installed
  • See if you can reproduce this if you copy the world to your computer and try to load it in single-player
  • See if you can reproduce this with a new world on the server

I can't reproduce it with a new world. I'll try the other two in a bit.
