Sometimes entities, player hand, player skin and outline that is on block you are looking on become invisible.

I originally suspected this mod myself, but it turned out to be MrCrayfish/Configured#70 for me.

You can easily reproduce it with Catalogue and Configured installed by opening the mods screen on the main menu and then going into a world. It seems to be dependent on the mods you have installed that Catalogue is trying to generate icons for.
