The first thing that came to mind when I thought of adding this to my modded playthrough is the swamp near my house, and the noisy mud blocks. It'd be very fitting to lose sanity while trudging through the mud.
Cherry blossom leaves and flower fields on the other hand, would be beneficial to run on/through.

A configurable list of positive and negative blocks to walk on would be a great addition to the mod, I think.
