Store the location and possibly render the substitution icon in mid-air when you are more than a certain distance from it.

The substitution will after being activated have a period of around 1-2 seconds where if you take damage it will activate and negate the damage. If this activation period you don't take damage you will be teleported.

The mark location will allow you to teleport within a certain radius back to the marked location. If it is out of range it will be random.
