What happened?

If interacted with in a certain way, the slicer can delete items.

If an axe is held in your offhand and you interact with the slicer with an empty main hand, the axe disappears and is not put into the slicer.

This has caused lots of items to disappear on my server!

I cannot recreate this with only slice & dice and a minimal set of mods. For me (which is also the expected behaviour), tools held in offhand are ignored when right-clicking the slicer.
Can you send me your modlist and verify in a single-player world if you can reproduce it there?

another way the slicer can delete items is if it's broken with a wrench while there's a tool still in the slicer, that's how I lost a diamond axe
