Dying without being able to get my two other slime forms.

I'm using this mod on my SMP server, and it when I became a slime, it was working at first, I logged on today, and ended up dying, instead of getting my four heart form I immediately died, I then restarted my server and the issue still had the same results.
I had this same problem. By downloading the most recent beta not the green file I fixed this problem. However, it does not appear to fix itself on preexisting worlds as a slime (I did this with minimal testing so if otherwise, please inform me). Also you only appear to halve size once, but you still get 3 lives.
