I can't seem to find any information on the crash in the Client Log, and the Crash Report is not a crash report - as per the template states: crash reports are stored in the crash-reports folder, timestamped for when this crash happened. Launcher logs have no information on crashes.

My issue DO NOT generate a crash report at all !
This is all I've got :(

I went ahead and recreated your pack using forge 47.2.20 and the latest version of each mod specified, and had no issues travelling across dimensions.

The following mods on your list are out of date, try updating them and see if that fixes the issue:
amendments
croptopia
embeddium
inventory profiles
journeymap
sophisticated backpacks + core
supplementaries
watermedia

I tried to update some mods but nothing (Inclued your list, GizmoTheMoonPig)
When I try to create a new world, the bug is not here, but on my existing world yes.
I can't delete my existing world to just fix this bug :/

if this doesnt happen on new worlds theres not really anything we can do about it. Considering I could never reproduce it and it doesnt seem to be an issue with TF itself, rather some strange unreproducible conflict with other mods, im gonna close this
