Stack refill strange issue not replacing.

Minecraft version: 1.21.4
Modloader: Fabric
Forge version: 0.16.10
Environment: Multiplayer, self hosted server, Linux.
Mod name: Stack Refill
Mod version: 4.6
Description
So as I place blocks and it is being replaced by a new stack, there is no block placed. The stack is counting down as I place blocks, but there are no blocks placed.
I need to go in to the inventory and place the stack in the quickbar manually for it to work. Still the blocks I "placed" are gone.

same issue but i tested in vanilla singleplayer, mod works but when it comes to servers the stack replaces but blocks are placing but counts still goes down
The same issue has been present for several versions... I am currently on Java 1.21.5 with the Fabric mod pack Fabulously Optimized. I upgraded my server world recently from 1.21.4 and had the same issue. Server is running vanilla 1.21.5 currently, client is running FO 1.21.5. The last time I remember any of the stack refiller mods working was around 1.20.
