Description

On my self hosted modded Fabric 1.21.6 server this mod causes an item to be lost on death. Conditions seem to be a random item of count=1 gets moved from inventory to the mainhand at the deathscreen. The item doesn't drop and doesn't respawn with the player, deleting it. Disabling this mod fixed the issue. I cannot recreate this on a solo player world.
