When I try to hit any entities in creative, survival, or even adventure, my fist wouldn't move and it wouldn't hurt the entity. When I try to left-click a block, it wouldn't work so I am forced to hold it for it to work. Every time I join a game, it starts off with a 1.0 tick rate per second.

Try Reinstalling forge and tickrate changer

There's a default tickrate in the config. The game triggers input events in the tickrate. In low tickrates, the game doesn't always register those events.
