Graves seems to fail to spawn or spawns below the bedrock when you die on top of the nether (above the bedrock). Is it possible to get the grave to spawn above the nether if you died above?
Minecraft Version: 1.12.2
Forge Version: 14.23.5.2795
Tomb Many Graves Version: 1.12-4.2.0

I had this happen to me today in my survival world. Lost a lot of stuff. :( The particle to the death spot took me to xyz 0/0/0 and no grave there either.

I would have expected it to abort it's normal procedure and either spawn the grave at maybe the portal you went through or just drop the items like in vanilla if it couldn't place it for whatever reason. Definitely think it should spawn the grave on top though.
