Was using the Sacrificial Knife to put Blood into the Blood Magic altar.
Clicked a bit too much and Killed myself.

No grave spawned and when I used the restore command it was my previous death before this one that got restored.

I've had the same happen for me using TombManyGraves-1.12-4.0.3 as part of the SevTech Ages Modpack 3.0.8.
