there's universal graves, but even before we added it the issue is happening already. there's like a mod for extra accessory slots similar to trinkets, but i don't know the name because I didn't create the modpack

yes, on the provided backpack slot that is separate from the armor slot

Then it's most likely this mods' fault. I do not interfere with any backpack after death handling if you equip backpack in back slot. This mechanic is handled only by the mod that provides the slot

weird... when we were playing there was no grave that appears when we die. The problem must be on our end. Thanks for this
