What happened?

I had anvils stacked one on top of the other and when I vein mined them I only got one back as a drop, tested it more times and still only one drops.

I also have Apotheosis which makes anvils enchantable, might be useful.

How do you trigger this bug?

1. Have anvils close to one another (for vein mining to work)
2. Break them using vein mining (I have tab as keybind)
3. Only one drops
