On the FTB Revelation pack running on Minecraft 1.12.2, using a Bound Scroll to return to our homes Waystone from the Mining Dimension from the Aroma1997's Dimensional World mod, users are 'Suffocating in a wall'. The Gravestone mod we have also doesn't kick off by giving them a gravestone, but does drop a waypoint indicating they were at the exact spot they'd exit the Waystone at in a normal situation, height included.

Teleporting between Waystones normally, or using a Bound Scroll while in the same world shows no issues. Testing returning from the Nether did not kill players, but they did notice they had taken anywhere between 3 to 6 hearts of damage.

No modifications were made to any mod configs or mods themselves, the pack is exactly as issued for Revelation 3.0.1

Not sure why it happened in 1.12 and there hasn't been any further reports of it on later versions, so closing this since 1.12 is now no longer supported.
