Waystone in SMP randomly teleporting to nether roof

Describe the Issue
Maybe once or twice a week I'll go to the nether via our waystone and it teleports me to the nether roof.

Do you use any performance-enhancing mods (e.g. OptiFine) or custom server distributions (e.g. SpongeForge)?

It just happens occasionally, it might be just a mix of mods but every
great now and then I will go to the nether and it'll just force me on to
any higher levels above the way Stone and if there isn't any higher levels
and there's the nether roof it puts me there where there is no way Stone.
