What happened?

The Gravestone Mod sometimes voids random items upon collecting your grave, especially when your inventory is full, and the excess item being thrown about.

My suggestion is to switch to the Corpse mod for the time being. Not only does it provide a inventory to collect your items from (so they are not thrown everywhere or into nearby lava), but it also places items back into the appropriate slots.
