What happened?

Playing the game when, without warning, all quests are suddenly "completed" at once. This starts at the ME quests and seems to move down alphabetically. Can't find anything that actually causes it. Does seem to have a correlation with opening the quest book. Causes the server to crash for all players with the error Internal Exception: java.net.Socket.Exception: Connection reset.
