Supplementaries Quills Void Main Hand when Opened in Offhand and Duplicates Map

What happened?
Put Quill in off hand, hold unobtainium pickaxe in main hand, right click ... bell dings a few times, back and forth, both hands flash a bit, not stopping until player stops it by removing item from one of the hands, and then they both turn into maps, and main hand item is gone.
