What happened?

Initially, I was able to go to the mining dimension through the teleport pad and was able to mine and interact normally. I then died in the nether twice later and have since been unable to return to the mining dimension. I have quit game and tried in a brand new creative world and was still unable to get the teleport pad to work. Doubtful the deaths in the nether are related but I was just including notable information from when it was observed.
