On my server I'm seeing the following errors:

I have a Mekanism Digital Miner in the Mining Dimension. The miner itself is chunk loaded and is set to have a radius of 64 blocks (my understanding is that only the miner needs to be chunk loaded and it does its own loading of the chunks it is mining). This happened after the update of the modack from 1.21.1 to 1.21.2.

This happened on server not singleplayer.

do those errors show up when you load the mining dimension (by going there) and then leaving it?

did you enable always chunk loading in the ftbchunks config or did you leave it as "only when online" ?

you are using the FTB chunk loading, right?

I'm using FTB chunks and I had changed the setting so that it would load chunks even when no players of the team are online.

The above errors appear immediately after the server starts. I tried going to the Mining Dim and leaving. Upon entering the Mining Dim, I got this warning though

The dimension itself may not be being loaded before mekanism is trying to interact with it. might be an issue with ftbchunks, jamd, mekanism, c2me, or perhaps something else.

You could see if the problem persists when c2me is disabled, or see if another chunk loading option works, though they may have to be enabled in the configs (anchor upgrade for mek, ars ritual, etc)
