Not completely sure if this is actually how it goes (could be misreading what happened), but anyway.
Sometimes items equipped during death will disappear. In my experience, it occurs when you die with cosmetic armor equipped when you die, and/or while using the gravestone; cosmetic armor will appear in place of normal armor, and normal armor doesn't appear at all.

I'm not sure if it's replicable, but I've seen other people with similar issues. Sorry if this doesn't have enough details.

I wasn't able to reproduce losing items, but I did manage to reproduce the Cosmetic Armor issue :)
