Unable to apply more than two damage enchants

Describe the bug
Apotheosis normally allows you to apply all the damage enchants on a sword. This used to work fine too. However, in 0.4.x I've been unable to add more than two. Any two, doesn't seem to matter, but a third won't go on.
I've done a little testing so far by removing Better End (which has Anvils and I thought may be related to the issue) but that does not appear to be the cause.

Well, ok then, it's not working with only Apotheosis... Guess I'll report over there >.>
