As far as I can tell, this is a Minecraft bug. The entity used for keeping track of the current chunk simply disappears after rejoining, so it seems like it is not being saved properly for some reason.

As this was not reported by anyone else for a long time, I'm marking this as closed. If this happens again to anyone, please open a new issue.
