[Content Patcher] add support for mod tokens

Let mods define their own tokens for content packs to use.
For example, Json Assets might add a token like {{json-assets/item-id:bamboo}} that returns the item ID for the custom item name that could be used by content packs to add NPC gift tastes, etc.
