Hi,
I haven't yet figured out how to replicate this issue. It happened to me most often in dungeons, and just now I got it after killing an elite rare in The Whirring Field. I think trying to exit a delve post-completion by pressing the space key when the confirmation prompt shows up was also triggering this error - need to verify.
