Updated to 1.21.0 today, activated Balance talents and when switching to Moonkin form, the Tree of Life bar (5) is displayed instead of the Moonkin bar (7).

And now I remember why I avoided using [form:x] for states :)

[form:5] which is what I changed Tree of Life paging to, actually means "whatever the 5th button on your class bar is" and not "tree of life"

Odds are, the final form will be something like [form,nobonusbar], since all other forms have actual bonusbar mappings.
