Unusable - Complete clearing of all my keys during a raid

Using this addon for multiple characters. In the middle of a raid, the addon reset COMPLETELY and left me with no working buttons. Completely unusable and immediately deleting this.

What do you mean by reset completely, any screenshots or error messages?
