Windwalker - Touch of Death being recommended even when it's on cooldown

Before You Begin
- I confirm that I have downloaded the latest version of the addon.
- I am not playing on a private server.
- I checked for an existing, open ticket for this issue and was not able to find one.
- I edited the title of this issue (above) so that it describes the issue I am reporting.
- I am reporting an issue with the default priority included with the specialization (imported or edited priorities are not supported).
Describe the Issue
Sometimes Hekili recommmends Touch of Death - even when Touch of Death is on cooldown
I haven't been able to figure out when.
The picture is not from the exact same situation as the snapshot.

How to Reproduce
Monk Windwalker - I haven't been able to figure out when it happens. Doesn't happen all the time.

That's not Touch of Death. It's pressing Storm, Earth, and Fire again to fixate them on a single target.
