In AoE or ST it sometimes it happens that it recommends an random Flame Shock cast.
Even when there is enough of the debuff on an enemy mob

How to Reproduce

1. Run M+ Totemic build from wowhead
2. Hit the AoE dummies or ST dummy

Because you had nothing else to cast and you have flame shock filler enabled. You can turn the setting off but you would just be standing there auto attacking instead. Sometimes enhance needs to wait for resources like MSW.

And if you look at your sim, even it uses that filler flame shock about 10 times in 5 mins, better than doing nothing.
