I tested it on Dummy in Dornogal.
Vanish isn't recommended the first few seconds (holding 2 charges for to long) 1st snapshot
Vanish is shown, after nearly 1 second it disappears (2nd snapshot)

I am not sure if I'm overlooking something, but I only see one snapshot -- the one where Vanish isn't recommended. Do you have the one from immediately before, when Vanish was recommended until after you used Between the Eyes? That's what would give us a basis of comparison to see what the addon expected to see vs. what it could see after actually using the ability.
Snapshot Review
From what I can see here, at least one of the reasons Vanish isn't recommended is that you have Ghostly Strike disabled. Vanish checks that Ghostly Strike is ready, and either (1) it's on cooldown for real or (2) it's not ready because you have it disabled.
In the snapshot, you have ~2 seconds left on your Ghostly Strike debuff, so I'm assuming Ghostly Strike is on cooldown anyway.

This one would've required that you don't have the set 2pc or you don't talent for Ghostly Strike, and that Vanish is approaching full charges. It also would've fired if there were fewer than 8 seconds left in the fight, but that is not the case.
