In dungeons and raids, the nameplates above enemies heads often just show as a bar with the name text missing. This does not always happen, and is inconsistent about which enemies it happens on.

Some enemies in a pack may have names displayed, while others don't.

This does not happen for enemy players, I've only seen it in dungeons / raids.

Closing this - it was actually due to a plugin.
