Spell icons aren't updating for spells that change

For example, when (shadow priest) Void Eruption is used it should change to Void Bolt but in the 1.0.4 release it stays as Void Eruption. It was working correctly in the previous version.

I'm having the same issue with Revive Pet / Mend Pet on my hunter. Interestingly, the same button that exists in a default Flyout (flyout dragged to the bar) DOES change.
