Also what's the quest(s) you turned in before this one became available?

Yes, https://www.wowhead.com/cata/npc=40289/ysera seems correct.

I don’t remember which quest I did before. I was already very near the achievement for completing the zone though.

According to another plugin I am using (which is usually right about these things), one has to complete https://www.wowhead.com/cata/quest=25612/return-from-the-firelands or https://www.wowhead.com/cata/quest=25611/return-from-the-firelands before it becomes available.

That's exactly what i also noticed ingame. How come it didn't have tooltip for you?

It could also be that the quest becomes avaliable before those 2 ids, but the NPC that gives it only shows after those 2 ids... can also be a possibility, but i need to see which quest actually enables this one if it's not 25611/25612

Also, you can check your quest history in Questie -> My Journey and check the list bottom left

I strongly suspect the exclusiveTo is wrong. I remember doing the quest before doing 27874.

I just had a look into the journal. I completed 25985 before accepting 27874.

That makes sense, but i swear there was a reason i put that exclusiveTo there. Maybe i did the quests in a different order and something changed? Will remove that entry though
