Hide threshold lines when unusable w/config

Add more configuration options for threshold lines, similar to the potion lines for healers:
- Hide threshold lines when you are out of range
- Hide threshold lines when you have no appropriate target
- Hide threshold lines when the ability is on cooldown

- Configuration for number of seconds or GCDs until it is usable again (like mana potions)
