I wanted to create an icon reminder that only triggers for feral when tanking, based on defense rating.

possible solution

Trigger > Player/Unit Info > Character Stats > Defense and/or Defense Rating

possible alternative

Item trigger base on Wolfshead Helm as it is most likely BiS for the whole expansion.
Player Stats trigger based on stamina, health or armor but that would be inaccurate or needs frequent updates.
