If you have a quest chain a->b->c->d->e all you can see right now is that quest e that you accepted and have in your log shouldn't be possible.
It takes time and effort to walk back in the chain and find the first quest where the conditions do not match reality.
It would be great if Wholly would assist here and show you where in the chain you should investigate.
