You may want to add solar panels to the lander (I forgot to).

WARNING: When you're ready to pull the lander out of the adapter, manually open it then press 1 to decouple the CM/SM. Dock with the lander section and then press 2 to ditch the S-IVB stage and adapter. Not doing this will result in a bad mission.

Munar Lander has enough fuel to land an rendezvous without having to ditch the descent stage.
The S-IVB stage has enough fuel to make multiple course corrections.
The first stage has enough fuel to reach an apoapsis above 70km. The second stage has the fuel on its own for a trans-munar injection from apoapsis without a Kerbin parking orbit assuming the Mun is in the right place.
Docking sadly has to be done from the CM/SM because the LM has no RCS thrusters (you can always add them yourself but monopropellant on the LM would decrease its ability to land and return to orbit on the descent stage alone).
