Item frames can be attached to a block so that they are located inside a protected region.

To be clear, is said block inside or outside the region?

The block is anywhere the player has build permissions on. It could be in a region or in no region.
Problem is players can place item frames to this block in a way that it's placed inside a region they do not have build permissions.

Practical example from our server: Players have their own plots inside a city which they cannot modify.
They place item frames on the outside of their plot and then can't remove the item frame.

Not sure why they have plots they can't even modify, but I think I understand what you're saying now.

I'll try to clarify it a bit:
Its the ''city'' they cannot modify, and in the city they have their own plots which they ''can'' modify.
The problem is when they attach item frames on the border of their plot so that the item frame itself protrudes outside of their plot. When that happens they won't be able to remove it because it's not inside their plot.

The image attached shows the problem. The wooden wall is the border of my plot. Outside of it is the city.
Problem is the item frame placement, it's attached to my plot's block but is located in the city's region so I can't break it.
