It's a nice thing to have when you're low on health and are close to dying

This already mostly exists, just set "Reveal Health when" to "Low" or "Not Full".

I did consider adding multiple triggers (so you can have both "Low" and "On Change", for example), but that is not currently possible and requires major work. Keeping this issue open to track that.
